Job Summary: The Growing Together Home Visitor is responsible for providing evidence-based supports and services to young children, their parents, and other caregivers by addressing behavioral and developmental issues, regular well-child care, safe environments, secure attachments, and positive adult-child interactions. 

Qualifications

Required:

Job-related experience within the specialized field is required.  

Bachelors degree.

Valid driver's license for employment.

Successful completion of Defensive Driving course, provided by RCDC.

Background Clearance.

Preferred:

Masters degree in job-related area. 

Minimum of five-year work history in a job-related field. 

 Responsibilities

Essential Functions:

  1. Demonstrate commitment to valuing diversity and contributing to an inclusive working and learning environment.
  2. Assist with implementation of Growing Together, focusing on a whole baby, whole family model, blending developmentally appropriate early childhood practices with primary health care by engaging physical and mental health care clinicians in a team approach whenever possible.
  3. Communicate with the childs primary health care provider, including participation in well child visits when appropriate, in order to assist in communication of the child and familys needs and facilitate follow-through of health care providers recommendations.
  4. Utilize Partners for a Healthy Baby curriculum to plan home visits that include providing information and support related to child development, including developmental screenings.
  5. Assist caregivers in developing the knowledge and skills necessary to enhance their childs development in the areas of social interaction, attachment, self-regulation, positive behaviors, and activities of daily living.
  6. Provide educational materials for caregivers in a variety of formats that emphasize prevention and health promotion and takes caregivers preferred learning style into consideration.
  7. Utilize appropriate tools with child and/or caregiver with respect to overall well-being (ASQ-3, ASQ:SE-2, Maternal Depression Screening, and other grant- or program-required measures) and provide information about appropriate community programs and resources.
  8. Monitor progress and adjust approaches as appropriate to encourage progress toward child and family goals.
  9. Achieve accreditation in applicable levels of Triple P Positive Parenting Program and utilize model as needed.
  10. Engage in prevention and promotion activities designed to increase community and caregiver knowledge about the impact of early childhood experiences as well as services and supports available with the community.
  11. Collaborate with a variety of groups and/or individuals (e.g. caregivers, team members, partner programs, etc.)  to effectively communicate information, resolve issues, and provide services.
  12. Participate in team meetings to generate options for the implementation of evidence-based approaches, supporting colleagues, and problem solving.
  13. Prepare and maintain a variety of manual and electronic documents, files, and records within required timelines to ensure the availability of information as required for reference and/or compliance.
  14. Participate in a variety of trainings, meetings, conferences, etc. to convey and/or gather information relating to job functions and agency mission.
  15. Support agency public awareness activities through agency contacts, distribution of materials, making presentations, writing articles, attending events, hosting visitors, and attending meetings as needed.

 

General Responsibilities:

  3. Demonstrate excellent interpersonal skills.
  4. Maintain strict confidentiality as outlined by FERPA and HIPAA.
  5. Ability to work alone and in groups to provide services. 
  6. Assume responsibility for individual professional growth by staying current with research and practices.
  7. Demonstrate proficiency in effectively and regularly communicating with other employees of RCDC and clients.
  8. Complete all assignments timely, including communication with other staff and clients or contractors. 
  9. Adhere to all professional guidelines as outlined in agency policies. 
  10. Independently manage daily assignments and activities ensuring performance outcomes are achieved within budget, timeline, department, and agency objectives. 
  11. Follow all safety requirements as directed by RCDC.
  12. Perform other duties as assigned by the Executive Director or designee. 

Physical Requirements:

  1. 70 % sitting, 15% walking and 15% standing
  2. Lifting up to 30 pounds.
  3. Manage all job duties with mobility, agility, and dexterity.
  4. Sit/stand for long periods of time.
  5. Ability to drive in varying weather and geographical conditions.
  6. The job is performed under minimal temperature variations and in a generally hazard-free environment.
