BASIC FUNCTION/PURPOSE
            To provide early intervention services for families with infants and toddlers identified as eligible for Part C in a family centered, team based philosophy using evidence based practice.

                The employee performing the duties of this position is frequently required to walk, sit, talk, hear, bend, stoop, and reach.  The employee may be occasionally required to manually lift, move, or carry up to 35 pounds.  The employee is required to interact with staff and meet the demands of several different people.  The employee is responsible for the work output of others.  The employee occasionally works extended hours.  Vision abilities required by this job include close vision, distance vision, depth perception, and the ability to adjust focus.
           
PERFORMANCE RESPONSIBILITIES

  1. Must possess the ability to work with families who have an identified child 0-3.  Prefer ability to work in evidence based practice/primary coach model.
  2. The candidate:
    • Consider alternatives for intervention based on available evidence
    • Documents options based on available evidence
    • Monitors progress systematically, and adjusts interventions based on data showing effectiveness/ineffectiveness of interventions with families
    • Selects professional development activities that are supported by evidence in effort to continually improve professional knowledge and expertise through conference, reading, participating in study groups, etc.
    • Provides leadership on team by including current evidence in discussions and planning activities
    • Assess developmental level of children birth to 3 years using appropriate assessment tools, behavior scales, observations, parent interviews, etc.
    • Work and plan collaboratively with the team of early intervention professionals and family as primary coach or secondary coach
    • Maintain continuous communication with parents through home and community visits, phone calls, e-mail, home activity sheets, parent meetings, etc.
    • Review progress toward IFSP outcomes, re-evaluate and summarize developmental progress, and revise IFSP’s within required timelines
    • Completes all necessary paperwork including, but not limited to: IFSP and supporting documents, data collection, Medicaid and regular logs, assessment summaries
    • Carry out the role of Primary coach/Family Service Coordinator to assist families with accessing needed resources
    • Work collaboratively with area agencies for identification and service delivery to eligible infants and toddlers
    • Attend staff meetings, in-services, and conferences as required by the Coordinator/district
    • Assist in community screenings, parent meetings, and other find activities as requested by Part C Coordinator
    • Assist with successful transition for eligible children into appropriate services at age 3
    • Travel as needed to over service area
    • Demonstrate skills for accessing technology for communication and completion of job-related tasks
    • Other duties as assigned by coordinator

 
Term of Employment:             School Year contract plus extended year days during the summer as determined by Part C Coordinator.  Must be available for early evening appointments as needed.
 
Salary Range:                          Determined by placement on teacher salary schedule.
 
Evaluation Frequency:            Job performance will be evaluated a minimum of 1 time per year.
